Heather, our Care Coordinator Nurse works closely with Doctors and patients to help identify their goals to support their chronic disease management, along with coordinating services and providers in order to meet those goals.
These goals may include:
- Health, well being, and early intervention/prevention
- Positive aging and disability services
- Linking the patient to required specialist assessment and services
- Ensuring consistency and continuity in the client’s care
